Question : How to create repeating pages in Word

I have a document in Word for retiring old files across our network. This needs to be filled in electronically and kept as a single document. I have created the first page but want Word to repeat this page each time we need a new one so that we can just keep adding to the document as we go. The only way I have found that I can do this is by inserting the text into the header, but obviously as soon as we fill it in once that information will be on every following page etc etc. Does anyone know how to make Word repeat the text across each consecutive page automatically?
If that doesnt make sense ask questions as I found it quite hard to explain!!! I have attached the file for reference.

Hi, if I understand the problem correctly, you want the table and the header to repeat on a new page. I would suggest recording a macro command to put in a pagebreak and create this table. Then assign a button for the command. this way whenever you want to add a new form just click that button and the new form gets created.
