Question : Veritas not detecting tape in drive

Windows SBS Server Veritas SBS Suite version 9.1 Revision 4691 HP DAT72 tape drive (6 months old, as are the tapes) Approx 20GB being backed up, 2 week rotation of tapes
Veritas does not always register the fact that there is a tape in the drive. Sometimes when a tape is inserted it will be detected and then listed in the 'Devices' tab under the tape device.
Recently the backups have been failing with the message 'Media Mount Failed' - I look in the morning and there is no tape listed in the device. The only way to ensure the backup runs every night is to insert the tape, run an inventory on the tape drive and then do a test run of the backup.
It has been gradualy getting worse and worse - HP have replaced the tape drive under warranty, there is no pattern to the media it affects, I have reinstalled Veritas (a few weeks ago when it first started to become an issue).
Any suggestions would be greatly appreciated!!

Weird.
CHeck the cables and termination as well as see if there are any Veritas updates/patches.
lastly - see if there are updated VERITAS drivers for the tape ( not hp Drivers ) !
