How do I set a user premission for the top level public folder (when I say top I mean when you look in outlook you have PUBLIC FOLDERS then + ALL PUBLIC FOLDERS) I want to be able to create a folder at the first public folder not under All Public Folders

You cannot, all public folders is a mapping to IPM_SUBTREE which is the root of the default MAPU PF tree, explanation:
Default tree referred to as the MAPI Clients tree, is the only tree accessible to MAPI clients. Each Exchange server in the organization has a default public folder store that points to this tree. In System Manager, the default name for this tree is Public Folders. In Outlook, you access this tree through the All Public Folders node.
