Question : Can I create a PDF form that allows user to insert their own picture?

I'm trying to create a attendee Q&A document as a PDF form. I'd like users to be able to fill out the form info (name contact info, etc). I'd like them also to be able to insert a picture of themselves.

This would allow us to easily print out all the completed responses without any after-filled-out editing. (Just print and go!)

Is this possible?
If so, how do I set that up? (I can't see how to do it in LiveCycle Designer)

Insert an Image Field that ought to do the trick. I opened up the employee sample application form and change the image for the logo to the image field object. When previewed and clicked on it allows the user to browse for an image file on their computer and replaces it.
