There is one or more disks missing in a volume group which has quorum checking disabled. The disk or disks are either faulty or are turned off. To check which disks it is, use "lsdev -Cc disk". If the disk(s) are turned off, turn them on and retry. Otherwise you will need to replace the disk(s) and fix up the config. You may also have to restore data from backup.
